The DAAD evaluation form has been formatted to my computer so that your evaluation is typed neatly and professionally with
all the information you wish to include to support your appeal. The more supportive detail you can provide the stronger
the evaluation. Most evaluations are two pages, mine average four.

MAST - The Michigan Alcohol Screening Test is a good measure of how serious a problem you have had with alcohol and-more
importantly-how well you accept that problem.

Go over the instructions regarding how the supporting letters and affidavit should be filled out as these
supporting documents are considered very important by the DAAD and can make or break your appeal.

Review what information on your Driving Record is important and why, and attach any relevant treatment records you
may have in your possession.

Discuss at length your involvement in AA or other support group that you rely on to maintain the lifestyle change
necessary to maintain permanent abstinence and your knowledge of the group's format.

Discuss the only diagnosis that will be helpful in getting your license back.

Referal to a local medical center to perform a ten panel drug screen with integrity variables, as required by the
state.

Discuss the importance of telling the truth and not trying to minimize your problem or bluff your way through the
hearing. The hearing officers are very astute and will not be fooled by "blarney." And discuss the importance of
being able to back up any claims you may make with documentation whenever possible.

Offer of a referral to a capable attorney to represent you if you so request.
